NMC Sponsored Event Reimbursement Form

Please use this form to request travel or purchase in advance reimbursement associated with conferences/workshops organized by NMC.

All travel or purchase must be approved in advance.  If you haven't received approval, please discuss with NMC Conference Manager Shannan Yeager (syeager@newmexicoconsortium.org).

Travel costs are reimbursed per federal government policy at federal government rates. Travelers are expected to make their own travel arrangements and submit the receipts for reimbursement within 30 days after the travel has been completed.

  • Hotels. Hotel costs are reimbursed at conference/group rate only. Taxes on the base conference rate are reimbursed. Other hotel charges will not be reimbursed.  Hotel costs are reimbursed after the travel and only with receipts.
  • Mileage. The current rate for federal mileage reimbursement is $.55/mile. We reimburse for the distance between your home institution and the conference venue unless other arrangements are made and described in the comment field. No receipts are required for mileage reimbursement.
  • Transportation. Transportation by bus, rental car, plain, train or other public transportation is reimbursed at standard fares with receipts only.  Insurance on car rentals is not reimbursed.
  • Meals. When we reimburse meal costs we do it through a meal per diem. No receipts are required to receive meal per diem.  However, per diem is reimbursed by exception only.  You don't need to request per diem.  You will receive meal per diem if your conference allows it and it has been approved in advance.
  • Conference Fees. If you have been awarded conference fee reimbursement, use this form to request payment. Please attach a receipt.
If you requested payment by check, please provide the address where you would like to receive the check.
Please tell us your itinerary and unusual circumstances (changes in origin, destination, shared rides).
Enter the number of hotel nights you submit for reimbursement. Hotel expenses will be reimbursed at the government rate plus tax. Submit corresponding hotel receipts below. No reimbursement will be issued without receipts.
Please enter the type of transportation reimbursement you are requesting. Attach copies of corresponding tickets, receipts and other supporting documentation below. If an individual expense is less than $25, you do not need to provide a receipt. Check the appropriate box and explain your reimbursement request and the amount in the comments field. To calculate mileage we use the distance between your home institution and the activity. No receipts required. Do not attach your own calculation of mileage.
If you are requesting reimbursement for conference fees, please attach the receipt(s).
NMC only reimburses per diem if it is allowed by the grant. In general, per diem is not reimbursed. We check the terms of each award to determine if per diem is allowed. Request per diem in the comments field below.
Please list the items that have been approved to be paid for the conference/workshop here. Attach your receipt below.

Attach receipts below. Please name the file indicating purposes of the receipt, e.g., receipt for hotel nights, receipts for rental car. Supported formats are doc, docx, pdf ONLY.
